OUR Home-Based-Care Programme
Dream Weaver Organization in collaboration with Networking Positive Women for Development (NETWOMENPLUS) provides
comprehensive Home -Based -Care (HBC) services, which includes health and social services by trained Models of Hope caregivers in the home to Persons Living with HIV and AIDS (PLHIV) who are beddridden or weak. Our Home-Based-Care programme provides physical, psychosocial and spiritual care. The service also focuses on reducing stigma and discrimination, prevent further spread of HIV and to mobilize community resources for People Living with HIV and AIDS (PLHIV) and their families, provide hope and comfort through good quality and appropriate care that helps patients and families improve the quality of life. Our HBC extends from health facilities to the home and vice versa.
The Aims and Objectives of Dream Weaver Organization:
1. Advocating and ensuring Women, Children and their communities can access portable drinking water and sanitary facilities all year round: Also Provides education to girls with good water, sanitation and hygiene facilities keeps them healthy, safe and in school.
2. Promoting Behavioural Communication Chanage in Malaria and TB endemic communities.
3.Reducing extreme poverty experienced by vulnerable women, Disable Persons, Persons living with HIV and Youth: We build capacity and provide empowerment schemes with support from our partner organizations and individuals towards changing the lives of vulnerable and deprived populations.
4.Prevention of HIV and STI New Infections among Key Population (FSW’s) and their Non-Paying Partners and General Population through promotion of Abstinence and Sexual & Reproductive Health (SRH) education among the youth, Mutual faithfulness, the correct and consistent use of condom. We also provide HIV Testing Services (HTS) and STI screening and treatment information to our target population.
5.Promotion of civic right and social accountability: We build capacity of citizenry on governance and civic right in project districts in order for community members to demand accountability from their leaders.
